The following Affiliation Nomination details can be completed & submitted online, complete the form & hit the submit button at bottom of page.

All Associations applying for Affiliation with the Western Australian Darts Council Inc must do so by completing this form, and posting a deposit and the required documents by the 31st March 2009, with the full balance due and payable by the 30th June 2009.

Before submitting the form online, print out a copy with which you can then post with the required documentation & payments

When submitting this form online, your registration as an affiliate of the W.A.D.C. will be noted, when either the initial deposit or full amount is received c/w with required documentation, you will be notified to your acceptance as an affiliate of the W.A.D.C.

The following information must be posted separately for your Affiliation application to be successful:
1. A LIST OF ALL PLAYERS REGISTERED WITH YOUR ASSOCIATION FOR 2009
2. A COPY OF YOUR ASSOCIATION FIXTURES FOR 2009
3. A COPY OF YOUR CURRENT CONSTITUTION (if not previously supplied)
4. A LIST OF EACH REGISTERED MEMBERS 2008 SEASONAL AVERAGE
